How Do You Apply for an Oklahoma Medical Marijuana Authority Card?

As the 30th state to legalize the use of medical marijuana, Oklahoma has set laws to be strictly followed, which includes securing a patient license. Patients who wish to buy medical marijuana from an Oklahoma dispensary need a Medical Marijuana Patient License.

This license, as mentioned by the Oklahoma Medical Marijuana Authority, “allows an individual with an approved application to legally buy, use, and grow medical marijuana and medical marijuana products in Oklahoma. This license will be in the form of an identification card that can be used to prove an individual is a license holder. This card will contain the individual’s name, photo, date of birth, city and county of residence, the type of license, the date the license expires, and the patient’s unique medical marijuana license number.”

Here are the steps you need to take to get an Oklahoma Medical Marijuana Authority License.

Step 1: Get an Oklahoma doctor’s recommendation.

The first step is to get a physician who is OMMA licensed and in good standing in the State of Oklahoma. The doctor will fill out and sign the Physician Application Recommendation Form. A digital copy of the form should be submitted with the application via the OMMA online system. This signed form must be dated within 30 days of the date you apply.

Note that not all doctors in Oklahoma state are licensed or comfortable in giving their recommendation for medical marijuana.

Step 2: Apply for Oklahoma Medical Marijuana Authority Patient License

Application and payment will be done through an online system on the OMMA website, so make sure to have a valid email address.

You will be needing to submit pertinent documents. Make sure that they are valid and unexpired.

  1. Proof of Oklahoma residency (one of the following, in digital copy)

  • Oklahoma ID card (front and back)

  • Oklahoma driver’s license (front and back)

  • Current rental agreement for residential property in the state of Oklahoma

  • Utility bill for the calendar month preceding the application, excluding cell phone or internet bills

  • Oklahoma voter ID card

  • Residential property deed to property in the State of Oklahoma

1. Proof of identity (one of the following, in digital copy)

- Oklahoma ID card (front and back)

- Oklahoma driver’s license (front and back)

- Tribal ID card approved for identification purposes by the Oklahoma Department of Public Safety.

-U.S. passport of the other U.S. government-issued photo identification

  1. Full-face digital photography

  2. The patient’s Medicare or Medicaid (SoonerCare) insurance card or enrolment documentation (if possible)

  3. Adult Patient Physician Recommendation Form dated within 30 days of Application submission.

Step 3: Secure your Payment For Your Oklahoma Medical Marijuana Authority License.

As stated in Section 1.D of State Question 788, “… The license will be good for two (2) years, and the application fee will be One Hundred Dollars ($100), or Twenty Dollars ($20) for individuals on Medicaid, Medicare, or SoonerCare. The method of payment will be provided on the website.”

The fee can be paid by Visa, MasterCard, Discover credit or debit card. An additional and non-refundable processing fee will be added to the transaction.

You must also take note that payments will not be refunded for applications that are not approved.

For individuals who do not have debit or credit cards, they can buy and use as transaction payment pre-paid Visa or MasterCard cards which are available in many retail stores and pharmacies.

Step 4: Wait for your Oklahoma Medical Marijuana Authority License Application’s Approval

OMMA will review your submitted application. If your application is complete and meets all the requirements, you will receive a letter of approval with your new ID, to be mailed within 14 days of your application submission date.

You will also be informed of unclear or missing documents through email stating the reason(s) it is found to be incomplete. Corrections can still be made by going back to your application and providing the needed documents. OMMA will honor the date you have submitted a complete application.

If an application is denied due to all requirements not being met, a denial letter will be sent which will include the reason(s) for the denial, and this will be mailed 14 days within the submission date.

Other important things to take note of when you finally acquired an Oklahoma Medical Marijuana Authority license:

  • Your license, unless revoked by OMMA for any reasons, shall be valid for two years.

  • Every time you purchase medical marijuana or any product related to it, you should present your medical marijuana ID card and another form of identification
